ABSTRACT

Introdução: Os acidentes por lepidópteros são agravos de interesse à saúde pública e a avaliação de suas características pode contribuir para melhorar sua assistência e prevenção. Este estudo analisou o perfil epidemiológico dos acidentes por lagartas do gênero Lonomia no Paraná. Métodos: Trata-se de estudo retrospectivo de base secundária realizado no Centro de Informação e Assistência Toxicológica do Paraná (CIATox/PR), em Curitiba, no período de 2015 a 2019. A análise foi realizada por estatística descritiva e Teste do qui-quadrado (p<0,05). Resultados: A amostra foi composta de 84 acidentes por Lonomia sp. com predomínio em adultos (45,2%), do sexo masculino (59,5%), e de lesões em membros superiores (84,5%). O contato com as lagartas ocorreram na área peridomiciliar da residência habitual do paciente (72,6%), na zona rural (64,27%) da macrorregião oeste paranaense (38,1%). O verão concentrou 71,4% dos casos, seguido por primavera e outono com 14,3% dos casos, respectivamente, sem registros no inverno. As manifestações clínicas mais frequentes foram dor local (62,4%), equimose (34,1%), eritema (34,1%), edema e queimação (17,6% cada). A classificação leve mostrou uma redução entre a fase inicial e a final do processo, ao contrário da moderada e grave, que tiveram aumento significativo de casos (p=0,006). O RNI (Relação Normatizada Internacional) foi incoagulável em 16,7% dos pacientes. Dois casos apresentaram piora do quadro clínico, sendo uma evolução para acidente vascular cerebral hemorrágico e um óbito. Discussão: Maior número de casos registrados na zona rural, tanto na residência habitual ou ambiente laboral, se dá pelo maior contato com o habitat das lagartas como árvores frutíferas e grandes monoculturas, o que também explica o fato de a macrorregião oeste ter o maior número de casos registrados. O aumento do desmatamento, incentivos à construção de parques públicos e plantio de árvores frutíferas na zona urbana são hipóteses para o aumento de casos nessa área. Percebeu-se que manifestações clínicas discretas podem ter uma evolução desfavorável, quando comparado estadiamento inicial e final. A realização do RNI se mostrou de grande importância na mudança de estadiamento, conduta terapêutica e diagnóstico. Conclusão: O perfil encontrado foi de acidentes por Lonomia sp. com sazonalidade no verão, em homens adultos da zona rural com gravidade moderada. Esses achados revelam a importância do diagnóstico e tratamento precoce frente às diferentes possibilidades de evolução clínica desses acidentes. É fundamental estimular estratégias para identificação da lagarta, notificação dos casos e medidas preventivas permanentes para reduzir riscos e agravos (AU).


Introduction: Lonomic accidents are of public health interest and the evaluation of their characteristics can contribute to the improvement of their attention and prevention. This study analyzed the epidemiological profile of Lonomic accidents in Paraná. Methods: This study is a retrospective secondary-based study carried out at the Center of Toxicologic Information and Assistance of Paraná (CIATox/PR), in Curitiba, from 2015 to 2019. Descriptive statistical analyses were performed using the Chi-square test (p<0.05). Results: The sample consisted of 84 accidents with Lonomia sp., showing greater occurrences among adults (45.2%) of the male gender (59.5%) and injuries on the upper limbs (84.5%). Contact with the worm occurred mainly in the patient's usual residence (72.6%), in the rural area (64.27%), and in Paraná's West macro-region (38.1%). Most cases were registered in the summer (71.4%), followed by spring and autumn (14.3%), and no accidents were registered in winter. The most frequent clinical manifestations were local pain (62.3%), ecchymosis (34.1%), erythema (34.1%), swelling and burning (17.6% each). The mild classification showed a reduction between the initial and the final phase of the process, as opposed to moderate and severe classifications, which showed a significant increase in the percentage (p=0,006). The INR was incoagulable in 16.7% of patients. Two cases showed progressive worsening, one of them evolved into a hemorrhagic stroke and one death. Discussion: The greater number of incidents registered in the rural area, both in the patients' usual residence and work environment, can be due to the greater contact with the caterpillar's natural habits, such as fruit trees and large monocultures. This also explains the larger number of registered incidents in the western macro-region. The increased deforestation, the construction of public parks, and the planting of fruit trees in the urban area are hypotheses for the increase of reported cases in this area. When comparing initial and final staging, it was noticed that discrete clinical manifestations could have an unfavorable evolution. The INR request proved to be of great importance in the management, diagnosis and change of staging of the patient. Conclusion:The epidemiological profile was of Lonomia sp. accidents happening in summer in adult males in the rural area with moderate severity. These findings reveal the importance of early diagnosis and treatment in view of these accidents' different possibilities of clinical evolution. It is essential to encourage strategies for identifying the caterpillar, reporting cases, and implementing permanent preventive measures to reduce risks and complications (AU).

ABSTRACT

Arsenic tops the list of priority list of hazardous substances 2022. People are frequently exposed to the environmental pollutant metalloid arsenic through their food, water, air, and soil. Arsenic is famous for its toxic effects. However, arsenicals have recently gained attention due to promising clinical trials for the treatment of acute myeloid leukemia. Presently numerous studies on arsenic's anticancer effects have been done. So a review was conducted on the use of arsenicals as poison and medicine. Arsenical was used as a powerful medicine in the BC era. But arsenical became famous as "the King's Poison" and "the Poison of the King" due to its use as poison. But the development of Marsh’s Test in the 18th century led to a decrease in the use of arsenicals as poison. In the 18th century, arsenicals were used as medicine to treat a variety of diseases such as fever, rheumatism, psoriasis, and syphilis. The development of antibiotics, new, safer chemotherapeutic agents, and radiotherapy halted its use as medicine. The dose and form of arsenicals make them medicine or poison. Many literary works reveal that arsenic's journey as “medicine” and “poison” is still ongoing in the twenty-first century.

ABSTRACT

Objective: To investigate the effects of duration, temperature and shake on paraquat (PQ) concentration in the blood of PQ-exposed rats during the specinen preservation and transportation. Methods: In March 2021, 60 SD male rats of Specific Pathogen Free class were randomly divided into low-dose group (10 mg/kg PQ) and high-dose group (80 mg/kg PQ). Each group was divided into 5 subgroups (normal temperature group, cold storage group, 37 ℃ storage group, shaking on normal temperature group and shaking on 37 ℃ group), six rats in each subgroup. The rats were given intraperitoneal injection of PQ, 1 h after exposure, the blood samples were obtained by cardiac extraction. After different interventions, the concentrations of PQ were detected and compared before and after the intervention in each subgroup. Results: In the shaking on 37 ℃ group, the results of PQ concentrations in PQ-exposed rats were significantly lower than those before the intervention (P<0.05). In the other subgroups, the results were not significantly different compared with before intervention (P>0.05) . Conclusion: The concentration of PQ in the blood of rats exposed to PQ was decreased by shaking for 4 hours at 37 ℃.

ABSTRACT

ObjectiveTo investigate the mechanism of Jiedu Huoxue prescription in promoting the reendothelialization of injured vessels by regulating the nuclear factor (NF)-κB/NOD-like receptor protein 3 (NLRP3)/cysteine-aspartic acid protease (Caspase)-1-mediated pyroptosis. MethodA rat model of injured thoracic aorta was established by balloon injury, and 36 rats were assigned into shame surgery, model, low-, medium-, and high-dose Jiedu Huoxue prescription, and atorvastatin calcium tablet groups. The injured aortic segment was collected 28 days after surgery. Hematoxylin-eosin (HE) staining and Evans blue staining were conducted to reveal the changes of vascular structural morphology and the reendothelialization of blood vessels, respectively. The enzyme-linked immunosorbent assay (ELISA) was employed to determine the levels of tumor necrosis factor-α (TNF-α), intercellular adhesion molecule-1 (ICAM-1), interleukin (IL)-1β, and nitric oxide (NO) in the serum. Western blotting was employed to determine the expression of endothelial nitric oxide synthase (eNOS), NF-κB p65, phospho-NF-κB p65 (p-NF-κB p65), NLRP3, and Caspase-1 in the vascular tissue. ResultThe model group showed thickened endovascular membrane, proliferation and disarrangement of smooth muscle cells of the artery wall, obvious inflammatory cell infiltration, and narrowed luminal area. Jiedu Huoxue prescription and atorvastatin calcium tablets mitigated the pathological changes of the thoracic aorta in different degrees. After balloon injury, the endothelial coverage rate of the model group decreased significantly, while Jiedu Huoxue prescription and atorvastatin calcium tablets increased the reendothelialization rate (P<0.05). Compared with the shame surgery group, the model group showed elevated levels of TNF-α, ICAM-1, and IL-1β (P<0.01) and lowered NO level (P<0.01) in the serum. In addition, the model group presented down-regulated protein level of eNOS (P<0.01) and up-regulated phosphorylation of pyroptosis-associated proteins NLPR3, Caspase-1, and NF-κB p65 in the vascular tissue (P<0.05, P<0.01). Compared with the model group, Jiedu Huoxue prescription and atorvastatin calcium tablets lowered TNF-α, ICAM-1, and IL-1β levels (P<0.05, P<0.01) and elevated the NO level in the serum (P<0.05, P<0.01). Moreover, the drugs up-regulated the expression of eNOS (P<0.01) and down-regulated the expression of NLRP3, Caspase-1, and NF-κB p65 (P<0.05, P<0.01) in the vascular tissue. ConclusionJiedu Huoxue prescription can promote the reendothelialization and inhibit the intimal hyperplasia of vessels after balloon injury by regulating the NF-κB/NLRP3/Caspase-1 pathway to inhibit pyroptosis and reduce endothelial inflammatory injury.

ABSTRACT

Background: Aluminium phosphide (AlP) is a common suicidal poison with a high mortality rate due to its cardiovascular impact and lack of antidote. The aim of the study wasto describe the electrocardiographic changes and other cardio-vascular manifestations in acute AlP poisoning and evaluate its impact on survival outcomes.Methods: A prospective cross-sectional study was conducted in a tertiary care hospital including patients who presented with acute AlP ingestion in any form. Patients’ vitals and ECG at the time of admission were taken and outcomes of survival were identified. A descriptive study of echocardiography was also done. The clinical parameters were correlated with the survival outcomes.Results: Fifty patients were included with 30 males and 20 females. The consumption of AlP in tablet form caused more hemodynamic compromise (hypotension and high anion gap metabolic acidosis) as compared to the powder form. ECG changes were seen in 42% of the cases, the most common finding was prolonged QTc interval (26%). The mortality was 30%.Hypotension, bradycardia, and QRS widening were found to be significant predictors of mortality (p<0.05). Echocardiography was performed for 10 patients, of which, one had right ventricular dysfunction and two had left ventricular dysfunction. Conclusions: AlP tablets are more lethal and hemodynamically compromising than AlP powder. Hypotension, bradycardia, and QRS widening are significant predictors of mortality. Direct cardiotoxicity leads to ECG changes, of which, QTc prolongation is the most common.

La mordedura de la araña de rincón es un motivo de consulta frecuente en los servicios de urgencia de Chile, que puede producir un cua-dro severo con manifestaciones cutáneas y sistémicas. En Chile, Loxocelles laeta se ubica principalmente desde la I a la VIII región, aunque se han reportado casos de loxocelismo en todo el país. El veneno de esta araña tiene efecto cutáneo-necrosante, hemolítico, vasculítico y coagulante. Podemos identificar 3 tipos de loxocelismo: cutáneo necrótico (80% de los casos), cutáneo edematoso (5%) y cutáneo visceral (10-15%). Este último tiene una letalidad entre 1 y 3% del total de casos de loxocelismo, la cual depende en gran parte de la precocidad de su diagnóstico y manejo oportuno. Se debe controlar cualquier tipo de loxocelismo durante las primeras 24 a 48 horas y vigilar la aparición de síntomas y signos sugerentes del cuadro visceral. No existe ningún examen de laboratorio que confirme el diagnóstico, los cuales sólo se alteran de modo marcado en los casos viscerales. El manejo de las lesiones cutáneas es con hielo local, antiinflamatorios, antihistamínicos y curaciones seriadas. En caso del loxocelismo visceral, el tratamiento principal es de soporte. La dapsona fue una indicación frecuente en el pasado y se asocia a efectos adversos graves, siendo el principal la exacerbación de la hemólisis, por lo que actualmente su uso no está recomendado. El suero anti-loxoceles no tiene evidencia que avale menor severidad ni mortalidad del cuadro.


The bite of the corner spider is a frequent reason for consultation in the emergency services of Chile, which can produce a severe reaction with cutaneous and systemic manifestations. In Chile, Loxocelles laeta is located mainly in the first to the eighth region, but cases of loxoscelism are reported throughout the country. The venom of this spider has cutaneous-necrotizing, hemolytic, vasculitic, and coagulant effects. Three types of loxoscelism can be identified, necrotic cutaneous (80% of cases), edematous cutaneous (5%), and visceral cutaneous (10-15%). The latter has a lethality between 1 and 3% of all cases of loxoscelism, which largely depends on the early diagnosis and timely management. Any loxoscelism should be controlled during the first 24 to 48 hours, and be alert to the appearance of symptoms and signs suggestive of visceral manifestations. There isn ́t any laboratory test to confirm the diagnosis. Laboratory tests are only markedly altered in visceral cases. The management of skin lesions is with local ice, NSAIDs, antihistamine and serial dressings. In the case of visceral loxoscelism, treatment begins with suspicion and early diagnosis. For these patients, the principal treatment is supportive care. Although it was recommended in the past, Dapsone is associated with severe adverse effects, like exacerbation of he-molysis, so its use is not currently recommended. The anti-loxocelles serum has no evidence to support less severity or mortality reduction.

Background: Since the start of the coronavirus disease 2019 (COVID-19) pandemic, poison centres worldwide have reported an increase in exposures to chemicals used for infection prevention. Increased availability and use could lead to an increase in exposures. Potential effects on a South African Poison Information Helpline were unknown, therefore a study was performed to describe changes in call volume and profile of poison exposures. Methods: A retrospective analysis was conducted on an observational database of telephone enquiries. All human-related poisoning exposure call data collected from 01 March to 31 August during 2018, 2019 and 2020 were extracted and analyzed. Summary statistics were used to describe all variables. Results: The total number of calls were 5137, 5508, and 5181 in 2018, 2019, 2020, respectively. The monthly call number during 2020 was mostly less than in 2019. More calls were received from the public calls (39.4% vs 33.1%) and for accidental exposures (65.6% vs 62.3%) increased during 2020 compared to 2019. Exposures to pharmaceuticals decreased by 14.8% from 2019 to 2020, while exposures to eucalyptus oil more than doubled from 21 in 2019 to 43 during 2020. Exposures to antiseptics and disinfectants increased by 60.4%, mainly due to hand sanitizers exposure which showed a 26-fold increase from 2019 (n = 6) to 2020 (n = 156). Conclusion: A change in the profile of poison exposures was observed during the COVID-19 pandemic. Lockdown regulations and greater availability of antiseptics and disinfectants probably led to the increase in exposures. Although symptoms were mostly mild, the public should be educated on safe storage and proper use of all chemicals.

ABSTRACT

ObjectiveTo investigate the mechanism by which Shenbai Jiedu prescription (SBJDF) inhibits the proliferation of colorectal cancer (CRC) HCT116 cells. MethodAfter 48 h treatment of HCT116 cells with SBJDF (0, 0.25, 0.5, 1, 2, 4 g·L-1), the viability of HCT116 cells were determined by methyl thiazolyl tetrazolium (MTT) colorimetry. Following the classification of cells into blank control group and SBJDF (1, 2, 4 g·L-1) groups, the effect of SBJDF on HCT116 cell morphology was observed under an inverted microscope. The effects of SBJDF on the proliferation of HCT116 cells and mitochondrial membrane potential (Δψm) were detected by colony formation assay and JC-1 probe, respectively. The flow cytometry was then performed for determining cell cycle distribution and apoptosis. The effects of SBJDF on cell cycle-, apoptosis-, and nuclear factor kappa-B (NF-κB) signaling pathway-related proteins were determined by Western blot. ResultSBJDF effectively inhibited the vitality of HCT116 cells and changed their morphology in a concentration-dependent manner. Compared with the blank control group, SBJDF at 1, 2, 4 g·L-1 significantly reduced cell colony formation (P<0.05, P<0.01),and SBJDF at 2 and 4 g·L-1 arrested the HCT116 cell cycle at G0/G1 phase (P<0.05, P<0.01). Compared with the blank control group, SBJDF at 1, 2, 4 g·L-1 remarkably down-regulated the protein expression of CyclinD1 (P<0.05, P<0.01). SBJDF at 2 and 4 g·L-1 lowered the CyclinA2 and cyclin-dependent kinase 4 (CDK4) (P<0.05, P<0.01). SBJDF at 4 g·L-1 reduced the cyclin-dependent kinase 1 (CDK1) (P<0.01). Compared with the blank control group, SBJDF at 2 and 4 g·L-1 induced HCT116 cell apoptosis, down-regulated the protein expression of anti-apoptosis-related proteins Bcl-2 and Bcl-xl as well as the NF-κB signaling pathway-related proteins IκB kinase α (IKKα),inhibitor α of NF-κB (IκBα),and phospho-NF-κB p65 (p-p65) (P<0.05, P<0.01), and diminished the mitochondrial membrane potential of HCT116 cells. ConclusionSBJDF inhibits the proliferation of HCT116 cells, which may be related to its inhibition of the activation of NF-κB signaling pathway and the induction of cell cycle arrest and apoptosis.

ObjectiveTo compare the feasibility of establishing the rat model of acute coronary syndrome with combined blood stasis and poison by lipopolysacharide (LPS) injection, ligation of coronary artery and different combinations of the two methods. MethodA total of 225 male SD rats were randomly divided into sham operation group, simple coronary artery ligation group, first injected LPS group [LPS(5 mg·kg)injection 24 h before coronary artery ligation] and follow injected LPS group [LPS(5 mg·kg)injection 10 min after coronary artery ligation]. The indexes of each group were detected at 3, 24, 72 h after modeling, and the model was comprehensively evaluated. The general state and macroscopic evaluation indexes of traditional Chinese medicine (TCM) syndrome (tongue and pulse) of rats in each group were observed. ECG and echocardiography were used to evaluate cardiac function, and the myocardial ischemia and infarction areas were measured by Evans blue/2,3,5-triphenyltetrazolium chloride (TTC) staining. The content of creatine kinase isoenzyme (CK-MB), lactate dehydrogenase (LDH), creatine kinase (CK), and troponin T (cTnT) in serum as well as interleukin-1 β (IL-1β) and IL-6 changes were determined by biochemical method or enzyme-linked immunosorbent assay (ELISA). Hematology analyzer was adopted to determine the white blood cell (WBC) count, and the four coagulation indexes, platelet aggregation rate, hemorheology and other coagulation evaluation indexes were also detected. The myocardial tissue was observed by hematoxylin-eosin(HE)staining. ResultAfter 3 h of modeling, compared with the conditions in sham operation group, the R, G and B values of tongue of rats (P<0.01), pulse amplitude (P<0.01), and cardiac function in simple coronary artery ligation group were decreased, and the color of hypoglossal veins became purple(P<0.01). The content of CK, LDH, cTnT, IL-1β and IL-6 in serum(P<0.05), myocardial infarction area(P<0.01), and total number of WBCs (P<0.05)were increased. Compared with simple coronary artery ligation group, first injected LPS group and follow injected LPS group had increased hypoglossal veins, decreased R value of tongue and elevated cTnT content (P<0.01), while follow injected LPS group had reduced B value of tongue, decreased cardiac output (CO)(P<0.05), increased IL-1β content, and thinned left ventricular anterior walls at end-systole (LVAWs)(P<0.01). After 24 h of modeling, compared with sham operation group, simple coronary artery ligation group presented significantly decreased R, G and B values of tongue, lengthened purplish dark hypoglossal veins (P<0.01), reduced pulse amplitude(P<0.01) and cardiac function, enlarged myocardial infarction area(P<0.01), increased whole blood viscosity, platelet aggregation rate, fibrinogen (FIB), shortened prothrombin time (PT) and thrombin time (TT)(P<0.01), and elevated total number of WBCs (P<0.01)and content of CK, LDH, cTnT and IL-6 in serum(P<0.05). Compared with the conditions in simple coronary artery ligation group, the pulse amplitude, R, G and B values of tongue (P<0.01), and ejection fraction (EF) and fractional shortening (FS) scores (P<0.05)dropped, and hypoglossal veins were deepened and lengthened(P<0.05), and cTnT content was increased(P<0.01)in first injected LPS group and follow injected LPS group. However, follow injected LPS group had thinned LVPWs, increased LDH content, platelet aggregation rate(P<0.05), myocardial infarction area, and total number of WBC, level of IL-1β(P<0.05), and shortened TT(P<0.01). Additionally, 72 h after modeling, compared with sham operation group, simple coronary artery ligation group showed significantly reduced pulse amplitude, lowered R, G and B values of tongue, thickened and lengthened hypoglossal veins(P<0.01), decreased cardiac function, and increased content of cTnT, FIB, whole blood viscosity(P<0.01),platelet aggregation rate, level of IL-6 and IL-1β(P<0.05). Compared with the conditions in simple coronary artery ligation group, the hypoglossal veins of the first injected LPS group and the follow injected LPS group were more purple, and the content of cTnT was boosted(P<0.01), whereas follow injected LPS group had decreased pulse amplitude, R, G and B values of tongue, EF and FS scores (P<0.05), and enlarged myocardial infarction area(P<0.01). ConclusionCompared with the other modeling methods and models at different modeling time, the established model by LPS injection 10 min after coronary artery ligation for 24 h was more consistent with the clinical characteristics of acute coronary syndrome with combined blood stasis and poison.

Hematology analyzer was adopted to determine the white blood cell (WBC) count, and the four coagulation indexes, platelet aggregation rate, hemorheology and other coagulation evaluation indexes were also detected. The myocardial tissue was observed by hematoxylin-eosin(HE)staining. ResultAfter 3 h of modeling, compared with the conditions in sham operation group, the R, G and B values of tongue of rats (P<0.01), pulse amplitude (P<0.01), and cardiac function in simple coronary artery ligation group were decreased, and the color of hypoglossal veins became purple(P<0.01). The content of CK, LDH, cTnT, IL-1β and IL-6 in serum(P<0.05), myocardial infarction area(P<0.01), and total number of WBCs (P<0.05)were increased. Compared with simple coronary artery ligation group, first injected LPS group and follow injected LPS group had increased hypoglossal veins, decreased R value of tongue and elevated cTnT content (P<0.01), while follow injected LPS group had reduced B value of tongue, decreased cardiac output (CO)(P<0.05), increased IL-1β content, and thinned left ventricular anterior walls at end-systole (LVAWs)(P<0.01). After 24 h of modeling, compared with sham operation group, simple coronary artery ligation group presented significantly decreased R, G and B values of tongue, lengthened purplish dark hypoglossal veins (P<0.01), reduced pulse amplitude(P<0.01) and cardiac function, enlarged myocardial infarction area(P<0.01), increased whole blood viscosity, platelet aggregation rate, fibrinogen (FIB), shortened prothrombin time (PT) and thrombin time (TT)(P<0.01), and elevated total number of WBCs (P<0.01)and content of CK, LDH, cTnT and IL-6 in serum(P<0.05). Compared with the conditions in simple coronary artery ligation group, the pulse amplitude, R, G and B values of tongue (P<0.01), and ejection fraction (EF) and fractional shortening (FS) scores (P<0.05)dropped, and hypoglossal veins were deepened and lengthened(P<0.05), and cTnT content was increased(P<0.01)in first injected LPS group and follow injected LPS group. However, follow injected LPS group had thinned LVPWs, increased LDH content, platelet aggregation rate(P<0.05), myocardial infarction area, and total number of WBC, level of IL-1β(P<0.05), and shortened TT(P<0.01). Additionally, 72 h after modeling, compared with sham operation group, simple coronary artery ligation group showed significantly reduced pulse amplitude, lowered R, G and B values of tongue, thickened and lengthened hypoglossal veins(P<0.01), decreased cardiac function, and increased content of cTnT, FIB, whole blood viscosity(P<0.01),platelet aggregation rate, level of IL-6 and IL-1β(P<0.05). Compared with the conditions in simple coronary artery ligation group, the hypoglossal veins of the first injected LPS group and the follow injected LPS group were more purple, and the content of cTnT was boosted(P<0.01), whereas follow injected LPS group had decreased pulse amplitude, R, G and B values of tongue, EF and FS scores (P<0.05), and enlarged myocardial infarction area(P<0.01). ConclusionCompared with the other modeling methods and models at different modeling time, the established model by LPS injection 10 min after coronary artery ligation for 24 h was more consistent with the clinical characteristics of acute coronary syndrome with combined blood stasis and poison.

@#Introduction: The objective of this paper is to investigate the behavioural intention to use the Poison Information System (PIS) among healthcare professionals based on the extension of the Technology Acceptance Model. Methods: A quantitative approach used a five-point Likert scale questionnaire adapted from previous research. Data were obtained from 136 health professionals working in the Emergency Department of public hospitals in Malaysia. Results: A multiple linear regression model showed that approximately 40% of the variation in intention to use was related to positive attitude, staff category, and inversely to computer anxiety. Perceived usefulness, perceived ease of use, and computer usage were related to intention to use, but their correlation was accounted for by positive attitude, staff category, and computer anxiety. A factor analysis grouped positive attitude, intention to use, and perceived usefulness on Factor I and perceived ease of use, inverse computer anxiety, and computer habit on Factor II. Cluster analysis indicated three clusters. Gender, age, experience, and staff category were strongly inter-related; intention to use clustered with perceived ease of use and perceived usefulness; positive attitude clustered with computer habit; and the latter two clustered together. Conclusion: These findings show that positive attitude, staff category, and computer anxiety of healthcare professionals, working in emergency departments, may have the greatest effect on PIS usage.

Abstract Introduction: Rhinella schneideri is a toad widely distributed in South America and its poison is characterized by inducing cardiotoxicity and neurotoxicity. Objective: In this work, we investigated pharmacological strategies to attenuate the peripheral neurotoxicity induced by R. schneideri poison in avian neuromuscular preparation. Methods: The experiments were carried out using isolated chick biventer cervicis preparation subjected to field stimulation for muscle twitches recordings or exposed to acetylcholine and potassium chloride for contracture responses. Results: Poison (10 μg/ml) produced complete neuromuscular blockade in chick biventer cervicis preparation within approximately 70 min incubation (times for 50 and 90 % blockade: 15 ± 3 min and 40 ± 2 min, respectively; P < 0.05, N= 5); contracture responses to exogenous acetylcholine and KCl were unaffected by poison indicating no specificity with postsynaptic receptors or myotoxicity, respectively. Poison (10 μg/ml)-induced neuromuscular blockade was not prevented by heparin (5 and 150 IU/ml) under pre- or post-treatment conditions. Incubation at low temperature (23-25 °C) abolished the neuromuscular blockade; after raising the temperature to 37 °C, the complete neuromuscular blockade was slightly slower than that seen in preparations directly incubated at 37 °C (times for 50 and 90 % blockade: 23 ± 2 min and 60 ± 2.5 min, respectively; P < 0.05, N= 4). Neostigmine (3.3 μM) did not reverse the neuromuscular blockade in BC preparation whereas 3,4-diaminopyridine (91.6 μM) produced a partial and sustained reversal of the twitch responses (29 ± 7.8 % of maximal reversal reached in approximately 40 min incubation; P < 0.05, N= 4). Conclusions: R. schneideri poison induces potent peripheral neurotoxicity in vitro which can be partially reversible by 3,4-diaminopyridine.


Resumen Introducción: Rhinella schneideri está ampliamente distribuida en Suramérica y su veneno es caracterizado por inducir cardiotoxicidad y neurotoxicidad. Objetivo: En este trabajo, investigamos estrategias farmacológicas para atenuar la neurotoxicidad periférica inducida por el veneno de R. schneideri en preparaciones neuromusculares de aves. Métodos: Los experimentos fueron realizados usando preparaciones de biventer cervicis de pollos sometidas a estimulación de campo para el registro de las contracciones musculares o expuestas a la acetilcolina y al cloruro de potasio para la respuesta contractural. Resultados: El veneno (10 µg/ml) provocó un bloqueo neuromuscular completo en las preparaciones después de aproximadamente 70 min de incubación (tiempos para 50 y 90 % de bloqueo: 15 ± 3 min y 40 ± 2 min, respectivamente; P < 0.05, N = 5); las contracturas en respuesta a la acetilcolina y el KCl exógenos no fueron afectadas por el veneno, indicando que no hay una interacción especifica con receptores postsinápticos o miotoxicidad respectivamente. El bloqueo neuromuscular causado por el veneno (10 µg/ml) no fue prevenido por la heparina (5 y 150 UI/ml) bajo condiciones pre y post-tratamiento. La incubación a bajas temperaturas (23-25 ºC) abolió el bloqueo neuromuscular; después de aumentar la temperatura a 37 ºC, el bloqueo neuromuscular total fue levemente más lento que el visto en preparaciones directamente incubadas a 37 ºC (tiempos para 50 y 90 % de bloqueo: 23 ± 2 min y 60 ± 2.5 min, respectivamente; P < 0.05, N= 4). Neostigmina (3.3 µM) no revirtió el bloqueo neuromuscular, mientras que 3.4-diaminopiridina (91.6 µM) produjo una reversión parcial y sostenida de las respuestas neuromusculares (29 ± 7.8 % de la reversión máxima alcanzada en aproximadamente 40 min de incubación; P < 0.05, N = 4). ABSTRACT Objective: To describe a case series of severe acute toxic exposures (SATE) in individuals <20 years old followed-up by a regional Poison Control Center (PCC). Methods: Descriptive cross-sectional study. All patients who were <20yo and classified as score 3 (severe) and 4 (fatal) following Poisoning Severity Score were included for analysis. According to the outcome, patients were classified as PSS 3 when they developed intense clinical manifestations with risk of death or important sequelae; and as PSS 4 when death had resulted from direct cause or complication of the initial exposure. The data of patients were obtained from the Brazilian electronic database system (DATATOX). Results: During the biennium 2014-2015, Campinas PCC followed up 5,095 patients <20yo, with 30 being classified as SATE (PSS=3, n=24; PSS=4, n=6). The exposures circumstances were unintentional (15); intentional (14; suicide attempt = 11; street drugs consumption = 3); and not explained (1). The exposures were significantly more frequent in adolescents >14yo (n=17; p<0.01). The involved agents were venomous animals (8; scorpions=5); medicines (8; miscellaneous=6); chemicals (6); illegal rodenticides containing acetylcholinesterase inhibitors (chumbinho, 4); drugs of abuse (3); button battery (1). Three patients evolved with sequels (esophageal stricture post-corrosive ingestion). The median length of hospital stay was 6 days (IQR: 5-12 days); 26 patients were treated in intensive care units, and 22 of them needed mechanical ventilation; 12, inotropic/vasopressors; and 3, renal replacement therapy. Conclusions: Scorpion stings and poisonings caused by medicines and chemicals were the main causes of SATE. The SATE were significantly more frequent in adolescents, due to deliberate self-poisoning.


RESUMO Objetivo: Descrever uma série de casos de exposições tóxicas agudas graves (ETAG) em pacientes <20 anos seguidos por um Centro de Informação e Assistência Toxicológica (CIATox) regional. Métodos: Estudo descritivo de corte transversal. Incluídos no estudo todos os casos classificados como escore 3 (graves) e 4 (fatais) de acordo com o escore de gravidade de intoxicações (poisoning severity score - PSS). Segundo o PSS, os casos são classificados em relação ao desfecho como escore 3 quando os pacientes desenvolveram manifestações clínicas intensas, com risco de morte ou que resultaram em sequelas importantes; e escore 4 quando a morte foi resultado de causa direta ou por complicação da exposição. Os dados analisados foram obtidos da base eletrônica brasileira do Sistema Nacional de Informações Tóxico-Farmacológicas DATATOX. Resultados: No biênio 2014-2015 o CIATox de Campinas atendeu 5.095 casos de pacientes <20 anos, dos quais 30 foram classificados como ETAG (PSS=3, n=24; PSS=4, n=6). Quanto à circunstância, 15 foram acidentais, 14 intencionais (11 por tentativas de suicídio e três por abuso de drogas) e um de causa não esclarecida, sendo significativamente mais frequentes nos adolescentes >14 anos (n=17; p<0,01). Os grupos de agentes envolvidos foram: animais peçonhentos (8; escorpiões=5); medicamentos (8; associações=6); produtos químicos de uso domiciliar/industrial (6); rodenticidas inibidores da colinesterase de uso ilegal (chumbinho=4); drogas de abuso (3); e bateria no formato de disco (1). Três pacientes evoluíram com sequelas (estenose esofágica pós-ingestão de corrosivos). O tempo mediano de internação foi de seis dias (mediana, quartis e intervalo interquartil=5-12 dias), sendo 26 pacientes admitidos em unidades de cuidados intensivos, dos quais 22 necessitaram de ventilação mecânica, 12 de inotrópicos/vasopressores e três de terapia de substituição renal. Conclusões: Escorpionismo e intoxicações por medicamentos e por produtos químicos de uso domiciliar/industrial foram as principais causas de ETAG, sendo elas significativamente mais frequentes em adolescentes, principalmente por tentativas de suicídio.

ABSTRACT Objective: To assess suicide attempts in children seeking care at a Poison Control Center. Methods: Cross-sectional study with children (<12 years old) that attempted suicide and were cared at the Poison Control Center in Londrina, Paraná, Southern Brazil, from April 1985 to December 2018. Results: We identified 59 children, most of them females (74.6%), who used only one product (77.9%). Among the products involved, medications were the most important ones (88.1%). Neurological/psychiatric/muscular manifestations (61.0%) were the main symptoms presented. The main reason identified for the suicide attempt was conflicts with family and/or friends (27.1%). Suicide attempts were more frequent in 2001-2003 and 2016-2018. Conclusions: Suicide attempts occurred mainly in female children with a single agent (mainly medications), and the main reason was family conflicts..


RESUMO Objetivo: Analisar as tentativas de suicídio em crianças atendidas em um Centro de Informação e Assistência Toxicológica (CIATox-Londrina). Métodos: Estudo transversal com crianças (<12 anos) atendidas no CIATox-Londrina, Paraná, de abril/1985 a dezembro/2018, com tentativa de suicídio. Resultados: Identificaram-se 59 crianças, com predomínio do sexo feminino (74,6%) e com o uso de apenas um produto (77,9%). Entre os produtos envolvidos, destacaram-se os medicamentos (88,1%). As manifestações neurológicas/psíquicas/musculares (61,0%) foram os principais sintomas apresentados. O principal motivo identificado da tentativa de suicídio foram os conflitos com familiares e/ou amigos (27,1%). Houve maior frequência de tentativas de suicídio nos triênios 2001-2003 e 2016-2018. Conclusões: As tentativas de suicídio ocorreram predominantemente nas crianças do sexo feminino, com um único agente (em geral, medicamentos), em que o principal motivo foram os conflitos familiares, observando-se um aumento ao longo dos anos.

Resumen Las medusas son organismos mayoritariamente marinos pertenecientes al grupo de los cnidarios, los cuales se caracterizan por presentar células urticantes especializadas, los cnidocitos. Si bien, todas las medusas son potencialmente venenosas y el grado de toxicidad depende de la especie, sus efectos sobre los humanos varían desde reacciones locales leves hasta reacciones atópicas-anafilácticas graves, llegando incluso a la muerte de la víctima. Mundialmente se conocen numerosas especies de medusas causantes de envenenamiento a humanos, tales como la avispa de mar (Chironex fleckeri), el sifonóforo carabela portuguesa (Physalia physalis) o el hidrocoral de fuego (Millepora spp.). En Argentina, tres especies de medusas revisten de im portancia clínica epidemiológica debido a su poder urticante: las hidromedusas Liriope tetraphylla y Olindias sambaquiensis, así como la escifomedusa Chrysaora lactea. Estas especies presentan sus mayores abundancias en el verano en las costas bonae renses, coincidiendo con la presencia de turistas durante el periodo vacacional. Sus afectaciones varían de leves a moderadas, registrándose desde parestesias y ardor con dermatitis, prurito, edemas y eritemas. Una infinidad de "remedios caseros" se conocen para remediar los efectos de las picaduras de medusas, sin embargo, la mayoría han resultado ineficaces y perjudiciales. Lo más recomendable es evitar frotar y lavar la zona afectada con agua dulce o aplicar hielo para tratar de contrarrestar el ardor y acudir lo antes posible al centro de salud más cercano.


Abstract Medusae are mainly a marine group belonged to cnidarians, which are characterized by specialized stinging cells, cnidocyts. Although all medusae are potentially poisonous, their toxicity depends on the species and the effects on humans vary- ing from mild local reactions to severe atopic-anaphylactic reactions, even the death of the victim. Numerous species of cnidar ians are known worldwide to affect humans, such as the sea wasp {Chironex fleckeri), the siphonophore portuguese man-of-war (Physaliaphysalis) or the fire-coral (Millepora spp.). In Argentina, three species of medusae are known with clinical epidemiological importance due to their stinging power: the hydromedusae Liriopetetraphylla and Olindias sambaquiensis, as well as the scypho- medusae Chrysaora lactea. These species have their highest abundances in the summer on Buenos Aires coasts, coinciding with the presence of tourists during the summer vacations. Its affectations vary from mild to moderate, registering from paresthesias and burning with dermatitis, itching, edemas, and erythema. An infinity of "home remedies" are known formedusae stings, how- ever, most of them have proven ineffective and harmful. It is best to avoid rubbing and washing the affected area with fresh water or applying ice to try to counteract the burning in the region and to go the health center as soon as possible.

Related records about discipline of diagnosis and treatment of exogenous febrile diseases by traditional Chinese medicine (TCM) were reviewed to summarize TCM knowledge about etiology, pathogenesis, progress and treatment of infectious epidemic diseases among exogenous febrile diseases. In consideration of onset time and major clinical manifestations of novel coronavirus pneumonia (NCP), this disease is considered as within the scope of pestilence in TCM, it is located in the lung, with etiology being poison of winter-warm syndrome, the viruses enter body via mouth and nose, and invade pulmonary collaterals. Based on the syndrome characteristics proposed in diagnosis and treatment schemes of various regions as well as the researches on medication rules, the present study points out the core pathogenesis of NCP is epidemic toxin invading the lung, pathogenic heat obstructing the lung and toxic heat blocking the lung. It also analyzes theoretical formula features of TCM Lianhua Qingwen and related clinical and experimental research results, in order to prove the clinical value of this drug in the outbreak of NCP.

Resumo: Introdução: As intoxicações por agrotóxicos causam elevada morbimortalidade. A vigilância é necessária da produção até o uso desses produtos. Objetivo: Analisar as intoxicações e os fatores associados à letalidade por agrotóxicos. Método: Trata-se de estudo caso controle baseado nos atendimentos de intoxicação por agrotóxicos realizados em 2017 por centros de informação e assistência toxicológica do Brasil. Pacientes que evoluíram a óbito compuseram o grupo caso, e os sobreviventes, o grupo controle. Calculou-se odds ratio (OR) dos fatores de risco para óbito, com intervalo de confiança (IC) de 95%. Com base no modelo de regressão, desenvolveu-se um modelo preditivo de morte, estratificado por faixa etária, sexo e contexto ocupacional, para investigação do risco dos trabalhadores agropecuários intoxicados por agentes extremamente tóxicos. Resultados: Identificaram-se 3.826 pacientes intoxicados por agrotóxicos, dos quais 146 evoluíram para óbito. Idosos (OR = 4,94; IC95% 2,49 - 9,80), homens (OR = 1,68; IC95% 1,15 - 2,46), trabalhadores do setor agropecuário (OR = 2,20; IC95% 1,15 - 4,24), tentativas de suicídio (OR = 13,27; IC95% 6,48 - 27,19) e exposição a produtos extremamente tóxicos (OR = 2,77; IC95% 1,84 - 4,16) apresentaram mais chances de óbito nas intoxicações por agrotóxicos. Conclusão: Em cada 100 intoxicações por agrotóxicos, quatro evoluíram para óbito. Idosos, homens, trabalho no setor agropecuário, tentativas de suicídio e produtos extremamente tóxicos apresentaram mais chances de óbito.


ABSTRACT: Background: Pesticide poisoning causes high morbidity and mortality. Surveillance is required for post-marketing monitoring of these products. Aim: To assess poisonings and associated factors with lethality by pesticides. Method: This is a case-control study based on the cases of pesticide poisoning assisted in 2017 by Brazilian Poison Control Centers. Patients who died were the cases and the survivors, the control. The odds ratio (OR) of death and 95% confidence interval (CI) were calculated. From the regression model, a predictive model of death was developed, stratified by age, gender and occupational context to investigate the risk of agricultural workers poisoned by extremely hazardous agents. Results: 3,826 patients poisoned by pesticides were identified, of which 146 died. Older people (OR = 4.94; 95%CI 2.49 - 9.80), males (OR = 1.68; 95%CI 1.15 - 2.46), agricultural workers (OR = 2.20; 95%CI 1.15 - 4.24), suicide attempts (OR = 13.27; 95%CI 6.48 - 27.19) and exposure to extremely hazardous products (OR = 2.77; 95%CI 1.84 - 4.16) odds of death from pesticide poisoning. Conclusion: Out of 100 pesticides poisoning, four died. Elderly, males, working in the agricultural sector, suicide attempts and extremely hazardous products had a higher risk of death.

La amalgama dental ha sido un material restaurador muy confiable, de gran utilidad en la odontología y que, aún con su gran aceptación y amplio uso en la profesión dental, ha presentado siempre muy limitado reconocimiento profesional, por su poca o nula capacidad estética o de color de diente y por las «guerras¼ que ha tenido que librar a través de sus muchos años de vida. El mercurio es un componente importante en la mezcla de la amalgama dental, y su presencia ha sido uno de los factores preponderantes que han causado el detrimento injusto del uso de este material restaurador, que ha demostrado un comportamiento clínico excelente en sus casi 200 años de vida (AU)


Dental amalgam has been a very trustable restorative material, highly utilized in dentistry and although with high acceptance and ample use in the dental profession, it has always had to deal with limited professional acknowledgement due to little or poor esthetic and lack of tooth color and because of the many «wars¼ it has to fight in the many years of life. Mercury is a very important component of the mix in the dental amalgam and its presence has been one of the key factors that has caused the unjustified detriment of the use of this restorative material that has shown an excellent clinical behavior in the almost 200 years of life (AU)

Background: Snakebite is a life-threatening medical emergency. It occurs frequently among rural people, especially those working in the fields. Most houses in rural areas of India are made of mud and have many crevices where rodents flourish. Delay in seeking medical aid or ignorance among primary care physicians about the correct treatment of snakebite is responsible for the high morbidity and mortality. Authors objective was to study the clinical profile of snake bite at a rural tertiary care centre.Methods: A hospital based cross sectional study was conducted at M S Rammaih Medical College Bangalore from June 2017 to June 2018. The incidence of Snake bite in Karnataka is 0.5% and at 95% confidence interval with Margin of error at 1% the estimated sample size was 197, but in this study author were able to collect data of 237 cases, hence all the cases were included in the study and analyzed.Results: Majority of the respondents were middle aged between 21-50 years. Around 63.3% of the respondents were male and nearly 56.2% were farmers. The incidence of snake bite of Cobra was seen in 8.9%, Krait 5.1% and viper 4.2%. Around 70% patients had no significant complications following hospitalization in our study. Around 20% had hematotoxic like ARF, DIC and local gangrene.Conclusions: Snakebite is one of the common hazards especially in rural setup as agriculture being the main occupation. Snake bite can present with various manifestations at bite sites, neurotoxicity, hematotoxicity.

INTRODUCCIÓN: Los productos de aseo y cosméticos son necesarios en el diario vivir y ampliamente utilizados por la población. Sin embargo, su uso puede no estar exento de riesgos, especialmente cuando no se utilizan o almacenan según lo recomendado. Es importante caracterizar las exposiciones, ya que eso es útil para implementar estrategias para reducir la morbilidad, mortalidad y costos asociados, especialmente en la población infantil. OBJETIVO: Caracterizar los reportes asociados a exposiciones a productos de aseo y productos cosméticos en pacientes menores de 12 años reportados al Centro de Información Toxicológica de la Universidad Católica de Chile (CITUC). PACIENTES Y MÉTODO: Estudio descriptivo transversal de llamados telefónicos reportados a CITUC durante el año 2016. Se analizaron las variables: edad, sexo, agente, interlocutor, localización del interlocutor e inciden te, circunstancia de exposición, vía(s) de exposición, sintomatología, y severidad, mediante fichas de registro manual y desde el software de registro electrónico denominado "INTOX Data Manage ment System" de la OMS. RESULTADOS: Cumplieron criterios de inclusión 3.415 casos. Los menores de 5 años representaron el 91% de las exposiciones y el 58,5% correspondió al sexo masculino. Un 99,4% correspondió a exposiciones accidentales y el 98,6% ocurrieron en el hogar. El 68,3% no presentó síntomas, tras la exposición. Las llamadas fueron realizadas por familiares (57%) y personal de salud (42%). Los 4 agentes con mayor incidencia fueron cloro doméstico (27,6%), limpiadores y lustres de pisos (13,1%), lavalozas (7,9%) y perfumes/colonias (5,8%). La principal vía de exposición fue digestiva (89,4%). CONCLUSIONES: Los productos de aseo y cosméticos son causas comunes de exposiciones especialmente en menores de 5 años. Si bien son productos de baja mortalidad y morbilidad, es importante educar a la población para prevenir posibles intoxicaciones en la población infantil.


INTRODUCTION: Household cleaning products and cosmetics are necessary for daily life and widely used by the population. However, their use may not be risk-free, especially when they are not used or stored as recommended. It is important to characterize exposures, as this is useful for developing stra tegies to reduce morbidity, mortality, and health costs associated, especially in the child population. OBJECTIVE: To describe reports associated with household cleaning products and cosmetics exposure in patients under the age of 12, reported to the Poison Information Center of the Catholic University of Chile (CITUC). PATIENTS AND METHOD: Descriptive cross-sectional study of phone calls to CITUC during 2016. The analyzed variables were age, sex, product, caller, caller and incident location, ex posure circumstances, exposure route(s), symptoms, and severity from manual records and from the WHO's electronic record software 'INTOX Data Management System'. RESULTS: 3,415 cases met the inclusion criteria. Children under the age of five represented 91% of the exposures, and 58.5% were male. 99.4% were accidental exposures, and 98.6% occurred at home. Family members (57%) and health personnel (42%) made the calls. 68.3% of the patients had no symptoms after exposure. The four products with the highest incidence were household bleach (27.6%), floor cleaners and polishers (13.1%), dish soap (7.9%), and perfume/cologne (5.8%). The main exposure route was by ingestion (89.4%). CONCLUSIONS: Household cleaning products and cosmetics are common causes of exposures especially in children under the age of five. Although these products have a low morbidity and mortality rate, it is important to educate the population to prevent possible poisonings in the child population.
